The employee must complete a Request for Accommodation form and submit official medical documentation as outlined in the instructions on the form.

Even if an employee has enough paid leave accruals to cover absences, if the need for leave qualifies as FMLA leave, this policy will apply.
As stated in Mercer's FMLA policy and in accordance with the Family and Medical Leave Act of 1993 (FMLA), as amended, Mercer University provides up to 12 workweeks of unpaid, job-protected leave to eligible employees for specified family and medical reasons during a rolling 12-month calendar or up to 26 workweeks for the care of a covered service member during a single 12-month period. The leave may be paid, unpaid or a combination of paid and unpaid leave as specified in this policy.
Employees must provide at least 30 days advance notice when the need for FMLA leave is foreseeable. When 30 days’ notice is not possible, the employee must provide notice as soon as possible.
If you are in need of leave under the Family Medical Leave Act (FMLA), please notify Nasana Graham by email or by phone at 478-301-2470.
